Why it Matters?
Gaps carry two distinct meanings that the analysis does not distinguish. A treatment interval with no records may indicate the claimant recovered, which is a damages argument, or that records from a provider were never obtained, which is a collection problem. Both matter and both require follow-up, but confusing them produces either a wasted argument or an incomplete file. The detection is mechanical; the interpretation is not.
